Wellness and Prevention
The Charter Health Plan supports preventive health care and promotes wellness activities. You and your covered family members should visit your PCP(s) within 90 days of enrolling to allow your PCP to become familiar with you, your medical history, and your current health status for future reference. Your PCP is available to advise you on ways to stay healthy and will work with you to understand the wellness care you need. The Charter Health Plan encourages plan members to consult their PCP about specific medical questions or concerns. It's important to keep in mind, there's an advantage to having a PCP who gets to know you and your medical history and coordinates all of your health care needs. The following preventive health care examinations and immunizations are recommended. The services listed below are covered (see Schedule of Medical Benefits in your Charter documents) for Charter Health Plan members and their dependents currently enrolled in the health plan:
 
Wellness Services for Adults

Once per year routine physical exam includes woman gynecological visit, mammogram, PSA test, lab and colon screening.  If you have had an annual physical with your elected PCP in the past 12 months, this requirement is waived. 

Well Child Visits

Physical exams appropriate for age of the child and recommended immunizations.  Immunizations are covered according to the Department of Health and Human Services Center for Disease Control and Prevention.  For recommended Childhood and Adolescent Immunization Schedules, click here:  Vaccine Schedules
