Groups that have been uninsured for at least six (6) months and in which the average employee earns no more than 250% of the current Federal Poverty Guideline.
Employees
All employees working at least 20 hours per week are eligible to elect medical coverage (your employer may have different minimum hour requirements for part-time status).
Dependents
Eligible employees who choose medical coverage for themselves may also elect coverage for their eligible dependents.
Spouse
An Employee’s spouse who is not legally divorced from the Employee or whose marriage to an Employee has not been otherwise legally terminated.
Children
An Employee’s unmarried children from birth through the age of 19 who reside locally.
An Employee’s unmarried children of any age who:
- Cannot hold a self-supporting job due to mental or physical disability and;
- Depend on the Employee for support and care. Dependency for support and care requires proof of incapacity and additional proof may be required at any time.
The term children means, biological children, step children, or legally adopted children who are claimed on an Employee’s tax return. To be eligible, the children must be in the Employee’s custody, be financially dependent on the Employee and reside locally. If the
children are not in the Employee’s custody, they may still be eligible if a court order or divorce decree requires the Employee to provide coverage and the children reside locally. |
