The Charter Health Plan website was designed with you in mind. It is intuitive, easy to use and informative.

Here you will find member information in many forms. We have wellness and prevention information resources on your left.

We have made it easier than ever to find a doctor, specialist or facility within your plan. You can also learn about your drug coverage and pharmacy locations near you. We have also provided you with important forms to download.

Caution
Prescription Reminder…… For your prescription to be filled at a local Davidson or Target pharmacy it must be prescribed by a primary care provider or specialist in the Charter Health Plan Network.  To see the list of network physicians click on the links for Primary Care Physicians or Specialists.
 
Newborn
For your newborn…… Your baby’s healthcare needs are covered under the Charter Health Plan for the first 30 days following the birth. However, if you have not enrolled your newborn in the Plan at the end of the 30 day period, coverage will cease. To prevent any break in coverage for your baby, please talk with your Employer and complete the Employee Change Request form as soon as possible. If your employer does not forward the completed form to the Plan representative, your baby will not have Charter Health Plan coverage after the first 30 days and will not be eligible until your employer’s next open enrollment period. In some cases this could result in months of waiting to obtain healthcare insurance for your baby. Please note The baby’s Social Security Number is not required at the time of enrollment. You can notify us when you receive the number at a later date. It is most important that we receive the Employee Change Request form before the end of the 30 day period.
