For your newborn…… Your baby’s healthcare needs are covered under the Charter Health Plan for the first 30 days following the birth. However, if you have not enrolled your newborn in the Plan at the end of the 30 day period, coverage will cease. To prevent any break in coverage for your baby, please talk with your Employer and complete the Employee Change Request form as soon as possible. If your employer does not forward the completed form to the Plan representative, your baby will not have Charter Health Plan coverage after the first 30 days and will not be eligible until your employer’s next open enrollment period. In some cases this could result in months of waiting to obtain healthcare insurance for your baby. Please note – The baby’s Social Security Number is not required at the time of enrollment. You can notify us when you receive the number at a later date. It is most important that we receive the Employee Change Request form before the end of the 30 day period. |
If you still have questions after reading the FAQ on the left, you will find contact info on our contact page. Call us and a member of our friendly staff will gladly assist you.
